INSTRUCTIONS FOR INITIAL CONSULTATIONS
To begin the process, please complete our Preliminary Questionnaire, which has been created as an online form. Simply click the button which you fill will find below to navigate to the questionnaire.
If you are unable to fully complete the online questionnaire, please make sure to save your progress so you can come back to it later. In that case, a special link will be sent to an email address of your choice that will enable you to resume.
Certain documents are requested at the end of the questionnaire. You can provide them in whatever manner you deem appropriate, whether email, postal mail, or delivery. Upon receipt of the completed questionnaire, someone from our office will contact you to schedule a convenient time for your consultation.
If there are two of you, we suggest you complete the questionnaire together, because some questions are designed for each of you to answer individually.
Here are three things we expect: (1) A fully completed questionnaire; (2) Complete, legible, originals or copies of existing documents; (3) Attendance by each person who is needed to make a “go forward” or “stop” decision.
